Bakarev M.A.. a series of fundamental studies was carried out to elucidate the regularities and features of tissue, cellular and intracellular reactions in somatic muscles, elements of the general concept of destructive-adaptive reactions of skeletal muscles in acute and chronic disorders of cellular metabolism of various origins have been developed: toxicometabolic damage by myotropic toxic agents, genetically determined metabolic myopathy. Priority studies of the morphogenesis of alimentary toxic paroxysmal myoglobinuria were carried out, known as the "Sartlan disease" and periodically occurring among the population of Western Siberia.

Installed, that structural restructuring of skeletal muscles in metabolic disorders is accompanied by a complex of adaptive processes and destructive reactions, which highlights the options, associated with acute and chronic dynamics of damaging effects. In acute injuries, the spectrum of reactions is dominated by universal types of damage to muscle fibers. (contracture, lytic and mixed), associated with characteristic changes in the myofibrillar apparatus. The nature of damage propagation depends on the structural-metabolic type and compensatory reserves of the target fiber and determines the form of damage. (limited or widespread), and, to a large extent, nature of reparative regeneration. An essential factor, determining the features of the structure and evolution of the lesion, is the integrity of the sarcolemma.

Distinctive features of acute destructive reactions in alimentary toxic paroxysmal myoglobinuria are a high degree of heterogeneity, as well as the predominance of limited and widespread forms of lytic lesions. A special variant of the necrobiotic reaction was identified in the ultrastructural picture (oncosis of the muscle fiber), in which intracellular edema, disposition and lysis of sarcoplasm structures are combined with moderate contracture changes.

The effectiveness of reparative regeneration of skeletal muscle fibers after acute metabolic damage was revealed in the experiment, which indicates the preservation of the proliferative and repopulative potential of myogenic progenitor cells, even with hereditary mitochondrial dysfunction. The concept of damage limitation strategy is formulated, elements of which are some of the manifestations of destructive reactions (fragmentation of the mitochondrial compartment, "mechanical" fragmentation and regeneration of the remaining parts of the muscle fiber).

In the context of the concept of the systemic nature of the structural response and the unity of alternative and compensatory-adaptive reactions of the mucous membranes Bakarev M.A.. structural and functional reactions of the gastroenteric system were studied in "local" and systemic pathological processes - chronic pancreatitis, opisthorchiasis, immune dependent granulomatosis, diabetes.
